Why this planner matters

Families aren't failing the transition. The system is hard to follow.

When a child with special needs approaches adulthood, the list grows fast: OPWDD eligibility, care coordination, self-direction, guardianship, Day Habilitation, SSI, CSE meetings, and more appointments than any calendar can hold.

For 15 years, I sat across from parents who were doing everything right and still falling behind. Not because they didn't care, but because the system was never designed to be easy to follow. The Transition Years Planner is everything I wished I could hand them. Roslyne D. Johnson, LCSW

Missed deadlines

School services end on a fixed timeline, and adult services take months to put in place. One missed date can mean a long wait.

The planner: weekly spreads and monthly calendars keep every date in view.

Incomplete applications

Eligibility paperwork, evaluations, and follow-ups arrive from different offices, often with no one tracking the whole picture.

The planner: step-by-step trackers for OPWDD, Care Coordination, Self-Direction, Guardianship, and Day Hab.

Confusion about services

Families are expected to understand acronyms and programs that professionals spend years learning.

The planner: a plain-language glossary of the terms you'll hear along the way.

Everything scattered

Notes from meetings, provider contacts, medications, and recommendations end up on scraps of paper and in a dozen email threads.

The planner: one place for every appointment, decision, and milestone.

What families say

“I received this planner as a gift and it is exactly what I need to help me stay organized and keep track of the many important developments in my child’s life.”
S.B., Parent, Long Island

Meet the author

Forty years beside families. One planner to carry them through.

I'm Roslyne D. Johnson, LCSW. For over 40 years, I have supported children and families as a Licensed Clinical Social Worker, and for 15 of those years I sat across from parents of students with disabilities as they faced the transition to adulthood. I saw the missed deadlines, the incomplete applications, the confusion about services, and how hard it was to keep everything organized. These were loving, capable families. The system is simply that challenging to navigate.

When I retired in June 2026, I put everything I had learned into one place: The Transition Years Planner, a 300-plus-page organizer that walks families through two full years of appointments, decisions, and milestones. Through Transition Years Academy, I now bring that same guidance to parent groups, schools, and agencies, so every family leaves with a plan and the confidence to follow it.
